BALLWEG FAMILY PHARMACY ∙ 1200 Prairie Street, Prairie du Sac, WI 53578 WHO: Vaccines are given to those on Ballweg’s waitlist. Prescription customers or anyone living in Sauk Prairie who is eligible based on WI’s current vaccination phase may be put on the waitlist.

HOW: Sign up for the waitlist at www.BallwegFamilyPharmacy.com. Ballweg Family Pharmacy will contact you via email and an automated phone call to schedule an appointment when it is your turn.

 

EANNELLI PHARMACY ∙ 405 Water St, Prairie Du Sac, WI 53578

WHO: Vaccines are given to those on Eannelli’s waitlist who are eligible based on WI’s current vaccination phase. Anyone who resides in the local area may be put on the waitlist.

HOW: Call 608-643-3396 to be added to the waitlist or fill out the form on Eannelli’s Facebook page. Eannelli Pharmacy will call you to schedule an appointment when it is your turn.

 

PRAIRIE CLINIC ∙ 112 Helen St., Sauk City, WI 53583

WHO: Vaccines are given to Prairie Clinic patients who are eligible based on WI’s current vaccination phase.

HOW:  No action is required; all Prairie Clinic patients who qualify are on tan internal list.  For patients age 65 and older, no action is required.  For other categories go to https://hipaa.jotform.com/210623335988158 to sign up on Prairie Clinic's waiting list.

 

SAUK PRAIRIE HEALTHCARE WELLSPRING ∙ 80 1st Street, Prairie du Sac, WI 53578

WHO: Vaccines are currently given to those who are age 65+ and on Sauk Prairie Healthcare’s waitlist.

HOW: Register for the waitlist at www.SaukPrairieHealthcare.org/COVID19-vaccine or call and leave a message at 608-644-3049. Sauk Prairie Healthcare will contact you to schedule an appointment when it is your turn.

 

WALGREENS PHARMACY ∙ 333 Philips Boulevard, Sauk City, WI 53583

WHO: Anyone who is currently eligible based on WI’s current vaccination phase may make an appointment.

HOW: Call 1-800-WALGREENS or create an account online at www.walgreens.com/findcare/vaccination/covid-19 to make an appointment. You will have to complete an eligibility screening.

Phase 1A

 Frontline health care personnel

Residents and staff in skilled nursing and long-term care facilities

 Police and fire personnel, correctional staff

 Adults ages 65 and over

 

Phase 1B (eligible as of March 1st)

 Education and child care

Individuals enrolled in Medicaid long-term care programs

Some public-facing essential workers

 Non-frontline essential health care personnel

Facility staff and residents in congregate living settings
