Matthew Grade, originally from Elm Grove, Wisconsin, attended Northwestern University in Evanston and Chicago, Illinois where he earned his Baccalaureate of Medicine degree and was in the Honors Program of Medical Education. He continued at Northwestern to earn his doctorate a few years later. Dr. Grade then completed his residency with the University of Wisconsin Family Practice residency program at St. Mary's Hospital in Madison, Wisconsin.

Dr. Grade moved to the Sauk Prairie area in 1980 when he joined the Black Earth Satellite Clinic. Then, in 1990, Dr. Grade joined Prairie Clinic. He was "attracted to the small town, rural setting that was close to Madison and the independent practice of comprehensive family medicine".
