Referrals from a Prairie Clinic Provider to see a Specialist

If your HMO insurance policy requires a written referral to see a specialist, the referral forms must be completed before you are seen. It is the patient's responsibility to contact our Referrals Specialist in order for these forms to be completed. Referrals to a specialist will often require an appointment with your Prairie Clinic provider, and should be scheduled well before your appointment date with the specialist.
