  When you come to Prairie Clinic for your annual physical, we ask that you fill out a history form to help keep your provider updated on your past and present medical history. The form for seeing our OB/GYN or Certfied Nurse Midwife are a bit longer, as they need more indepth information. The forms are posted below:

  • Female History Form (Female Physical - except Dr. Schad and Glenna)
