Prairie Clinic Participates in Wisconsin Immunization Registry

Prairie Clinic participates in the Wisconsin Immunization Registry (WIR) program. WIR is a statewide database of immunizations to help keep everyone on track for their recommended immunizations and provides that information to health care providers and schools.

Patients can look up their immunization records online by clicking on the Public Immunization Record Access. A WIR Parent Brochure is also available from the WIR site.

Parents will need their child's Social Security Number to access their child's records. If your child was born in Wisconsin after 1995, WIR will probably have your child's Social Security Number. If your child was born before 1996 or if you have trouble accessing your child's records, contact the clinic.
